Unguis incarnatus | Onychocryptosis
These terms are often used synonymously and all refer to an ingrown nail.
Other terms for this problem include “embedded nail” or “granulomatosis of the nail matrix”
Usually the big toe nail is affected laterally (on the side).
Causes
From a scientific point of view, the causes of ingrown nails are primarily divided into two main groups, although these factors can also occur together.
One examination of the causes focuses on the soft tissue surrounding the nail, another examination focuses on the nail itself.
Simply put, the causes are often the wearing of shoes that are too tight, injured or damaged organisms (traumas) and | or orthopedic misalignments, which lead to a physiological and | or
external pressure on the nail fold is triggered.
An increase in internal pressure at the edge of the nail can also be caused by an unfavorable nail cut and/or by inherited factors (hereditary).
An increase in pressure between the nail fold and the nail edge can generally lead to the formation of ulcers and suppuration of the nail fold.
An existing ulceration promotes the formation of skin germs.
In combination with poor foot hygiene, poor circulation and/or moist environments (sweating) and possibly also diabetes mellitus, the development of nail bed inflammation (paronychia) is likely.
The inflammation, in turn, causes swelling, the proliferation of easily injured hypergranulation tissue and thus leads to an “overgrowth” of the nail from one side or the other.
This in turn increases the pressure between the surrounding soft tissue and the nail edge.
A vicious circle (Circulus Vitiosius) that explains the often chronic or chronically recurrent course of the disease.
Orthonyxia
The term 'orthonyxia' refers to the use of metal wire braces to straighten an ingrown nail.
Arguments for braces treatment
The brace treatment

Emmert plastic surgery (surgical procedure)
The Emmert plastic surgery is the standard surgical procedure for the treatment of an inflamed unguis incarnatus and leads to a permanent aesthetic impairment of the nail, since the lateral third of the nail including the adjacent nail matrix (nail fold | nail wall) is cut out (excised) in a wedge shape.
After the (partial) nail and the tissue are removed, the bleeding is carefully controlled. The wound is then treated with a sterile dressing; the wound is usually not sutured.
Postoperatively, the wound area must be cared for by regular dressing changes and simultaneous wound monitoring.
As the effect of the local anesthesia wears off, severe pain will occur, which should be counteracted by taking a painkiller.
Note: Not every patient tolerates painkillers.
The full healing process usually takes about 14 days. During this time, incapacity to work (with high costs for the employer) is likely.
Duration of orthonyxia treatment
Orthonyxia treatment typically takes about 20 to 24 weeks, as the brace typically needs to be tightened four times at intervals of four to six weeks after the initial fitting. In exceptional cases, treatment may take longer.
With a three-part orthonyxia brace, such as our SSO4 braces instead of OP ® , the two brace legs can be reused up to five times.
The U-loop connecting the two legs of the brace, which is used to tension the legs, is destroyed with the help of a side cutter before the brace is reattached and must be replaced.
Material requirements for a complete orthonyxia treatment with the SSO4U brace instead of surgery ®
For a complete brace treatment, you typically need 1 brace and 5 U-loops.

Costs of the application | Contribution of the health insurance company
After you have informed your patient about the possibility of gentle nail correction using the SSO4U brace instead of surgery® and its advantages, you can give them the positive news that they may be able to have part of the costs reimbursed by their health insurance company.
The cost for the first fitting of the braces varies between 150.00 and 280.00 EUR depending on the region and, in our experience, the cost for refitting is between 20.00 EUR and 50.00 EUR depending on the effort involved.
These values depend on the location of the practice, as different costs arise depending on the location - a practice at Marienplatz in Munich has a different rent than a practice in a structurally weak area.
Assuming an average cost of EUR 30.00 for refills and a minimum of EUR 150.00 for the initial filling, we arrive at a total of EUR 270.00 for a complete treatment. Occasionally, a further refill is necessary, so your total bill will often be around EUR 300.00.
The cost of the brace material is a maximum of EUR 24.00 (without discounts). This gives you a margin of EUR 276.00 for a relatively small investment of time.
Compare the time you spend on such a margin with the time you spend on normal foot treatments.

Cost estimate for brace therapy
You can help the patient apply for a subsidy from their health insurance company by completing a cost estimate for them. You can either give this to the patient or contact their health insurance company yourself.
Patient contract
To protect yourself against any health risks and subsequent risks and to avoid working for free, you should draw up a patient contract with your patient and have the patient sign it.
Practical knowledge of billing – have follow-up payments paid privately
Because you may have to wait a long time for your payment when settling with your health insurance company - namely until the treatment is completed plus the health insurance company's processing time - we recommend that you settle the follow-up appointments in cash and provide a receipt, which the patient can then present to their health insurance company after treatment has been completed to claim reimbursement of the expenses.
